Clear data before customer creation

Please take care of this bug detailed in slack:

_When you click on the "Add new customer" button you do not get a clean/clear form to input your new data so the first thing you are going to worry about is "am I changing what is there now"? And you are going to click on the Save button again to be sure it is saved._
