Approval to send liaison statements

Individuals sending outgoing liaison statements need to have prior approval.

We need to:

• Insert a checkbox in the form that allows the user to indicate he/she has the approval
• If the user doesn't need approval the checkbox will be disabled
• If the approval checkbox is not checked:
  • The liaison will be hold in pending
  • Persons who must approve the liaison will be notified by email

Save approval state of an outgoing liaison and do not public list the not approved ones. Fixes #353
