Default state changes for new revisions don't make sense for all streams

Note the transitions to "AD Followup" on new revisions. This is an independent stream document - the notion of "AD Followup" does not make sense.

It would be better for the independent stream to just clear tags and put (leave) the document in the "In ISE Review" state.

Other non-IETF streams need similar adjustment.
