Currently the code automatically adds to the skip count for a reviewer when a review is assigned "out of sequence". This is not the right thing when the assignment is given because the reviewer had already reviewed a previous version (and probably for some other cases). It seems better to make whether to add to the skip count an explicit choice per assignment.
