When multiple teams are selected when requesting a review, only one review request is created

When an AD or chair hits the [Request Review] button, the resulting form has a multi-select widget listing the active review teams. If more than one team is selected, a request should be made of each selected team. Currently only one of them (not sure which is chosen) is created.

Save a new reviewrequest for each team requested in the request form. Fixes #2094. Commit ready for merge.
