Make it possible for a member of a review team to request being assinged a document while looking at the document

When someone is logged in and is a member of a directorate that performs reviews, the main page for a document and the rows for search results should make it easy to say "I want to be the team reviewer for this document when it comes time to review it".

I suggest something similar to adding and removing drafts from a personal community list (but taking into account that the user might be a reviewer for more than one team).

The workflow for this seems simple for people that are on a single review team. It'll be just as simple for them as tracking the document is now.

However, people on multiple teams will need an intermediate step to select for which team they'd like to add a review wish. Is that really more helpful to people than going to their review wishes and adding the document?

I'm asking because another option I had in mind is to only enable this for reviewers that are on a single team. This covers 80% of reviewers, which are also the reviewers for which this feature brings the largest benefits. All they need to do is click a button, whereas the others will need to select a team first, and then submit that.

However, I don't really know the reviewer workflow well enough to make a good estimates of how and when people use this feature - so I'm also happy to move forward with supporting it for all reviewers, optionally with an intermediate step.

Fixed in 6e55f26:

Fix #2050 - Allow adding review wishes from document and search pages.

On the main page of a document and in document search results, a new
button allows review team members to add a review wish for that document.

For reviewers that are only on one team, this essentially works
identical to tracking a document. Reviewers that are on multiple teams
are lead through an intermediate step to select a review team, and then
returned to their search or document page.

Commit ready for merge.

On a sidenote, the current review wish overview on a reviewer's "profile" page seems to list documents by ID, not name? This feels like a bug?
Should be a trivial fix, it's in ietf/templates/ietfauth/review_overview.html:98 which refers to w.doc_id now rather than w.doc.name.
