The interface uses Questionnaire to mean both the set of questions that a chair can edit (via a template) and send to candidates, and the answers that candidates send back. This is confusing, even to past Nomcom chairs.
The set of questions should be called a "Questionnaire".
The answers that come back need to be called something else consistently. I suggest "Questionnaire Response" following the terminology in RFC6730.
