Request affiliation process

User can request affiliation

• Create a button to allow Users to affiliate with organization. See ref1 for where the button could go. This placement aligns with the mytracker button and the on-demand scanning button.
• Log affiliation request in the activity log of the organization
• Insert some sort of rate limit for affiliation requests
• Send affiliation request email to org owner and org admins with link. Include no PII in the email. Include instructions in the email about how to 'invite user officially'.
• This is an auditable event when a admin accepts an affiliation request.
